    Mucinous tubule and spindle cell carcinoma (MTSCC) of the kidney is a rare renal tumor generally considered of low malignant potential. Radiological diagnosis of the MTSCC is frequently difficult as those tumors tend to present as more discrete changes than those typical for classical renal cell cancer. The radiological diagnosis of a MTSCC can be challenging. It is important to establish correct diagnosis preoperatively in order to qualify patients for NSS.

    The application of ICG in surgical fields of medicine – a short narrative review

    Background: Intraoperative near-infrared imaging using indocyanine green (ICG) has been increasingly used in procedural medical fields in recent years. The purpose of our work is to provide a general overview of the use of indocyanine green in procedural medical fields. We reviewed publications posted in PubMed, whose authors used ICG in their research, but also daily surgical practice. Many large studies have demonstrated the usefulness of this method. Currently, new directions for the use of this dye are being sought. In the future, the use of ICG in near-infrared (NIR) imaging may become a standard used in daily medical practice. Material and methods: A review of the available literature was conducted using the PubMed. The search terms ‘ICG’, ‘’indocyanine green’, ‘indocyanine green fluorescence’, and ‘NIR’ were used. Results: We analyzed several dozen articles from the large database available in PubMed. ICG is currently used in many different fields of medicine. Below we list the applications found in this review. Conclusions: Indocyanine green has demonstrated utility across various surgical disciplines within the medical field. A growing body of research has elucidated the multiple advantages conferred by this dye, thereby propelling its increased adoption by researchers and practitioners. Despite these advances, there remains a need for extensive research before ICG can be integrated into routine clinical applications.

    An Amyand hernia is defined as a vermiform appendix located within the inguinal canal. It is a rarely seen condition with complications that are even more uncommon. We report the case of an Amyand hernia containing a strangulated epiplon and vermiform appendix.

    Urethral Profilometry – should it be discarded?

    Background: Urethral profilometry was developed as a scientific research method rather than for everyday clinical use. However, it was implemented as an integral part of urodynamic diagnostics for clinical use. Although it has been used for over 70 years, there are still no clear guidelines, either for the technique or for the indications and interpretation of the data obtained in this study. There are many inaccuracies and doubts about the method. Material and methods: The authors of this manuscript present a review of the options for urethral function diagnostics and the position of urethral profilometry among them. The literature and clinical aspects of their investigation are discussed in this article. The sources of the limitations of urethral profilometry and their importance in clinical practice and the interpretation of results are analyzed. Results: Urethral profilometry has many limitations. Their sources are urethral anatomy and structure, equipment, patient collaboration and the examination technique. The repeatability is debatable, but doubts surrounding it seem to be exaggerated. Despite all this many valuable data can be obtained by performing an examination. Conclusions: Despite the limitations, urethral profilometry is a useful clinical tool for functional diagnostics of the urethra. Developing precise guidelines regarding indications, technique and interpretation of the assessment would be essential in order to take advantage of this diagnostic method.

    Acute small bowel obstruction due to Quain hernia (caused by defect of broad ligament), diagnostic and treatment challenge

    Background: Acute small bowel obstruction due to internal hernia is a rare condition. One of the most rare encounters is the Quain hernia (caused by broad ligament defect). Material and methods: A rare case of internal Quain hernia due to a broad ligament defect that caused an acute small intestine obstruction is described. Results: The challenge of diagnosis and the treatment choice is briefly discussed. Conclusions: Patients with broad ligament defect could be clinically very chellenging with not effective RX for the early diagnosis. CT scan with intravenous contrast enhancement could be useful for diagnosis and CT radiologic sign of deviation of the “healthy” uterus to the left or to the right side could be considered like an indirect CT sign of Quain internal hernia. More clinical case studies of treatment options are necessary for further evaluations.

    The impact of the SARS-CoV-2 pandemic on screening tests rates in breast and cervical cancer

    The global SARS-CoV-2 pandemic had a significant impact on the availability of screening for breast and cervical cancer. In Poland, two pandemic waves were observed in 2020, characterized by an increase in the number of confirmed cases and deaths caused by the SARS-CoV-2 infection. This situation has reduced the number of patients registering for mammography and cytological tests. The aim of this study is to describe the problem using the available numerical data obtained from the database of the regional branch of the National Health Fund regarding the significant decrease in the number of screening tests for breast and cervical cancer in the Podkarpackie Voivodeship during the SARS-CoV-2 pandemic.

    Patient with previously misdiagnosed total situs inversus

    Background: The case of 30 years-old patient with recently diagnosed dextrocardia is discussed hererin. Material and methods: Dextrocardia, a specific anomaly of heart position in the chest, usually associated with situs inversus and sometimes by Karteneger’s syndrome was diagnosed in the patient by careful physical examination proven by electrocardiography and imaging. Results: Specific ECG changes were crucial to recognize the syndrome. The patient presented total situs inversus in physical examination. The case is a good exemplification of frequent dextrocardia oversight, since it has not been found during previous 30 years by medical professionals, including both pediatrician and occupational medicine specialist. Conclusions: The attention should be always turned to make a proper diagnosis because it allows to recognize properly additional diseases as well as avoid misdiagnosis in future.

    Emergency surgery in the COVID era

    We describe the case of a COVID-positive patient with a clinical diagnosis of acute appendicitis who was a prisoner at a maximum security facility without access to a prison surgical ward. The administrative and legal issues related to admitting and treating this patient on a standard surgical ward are briefly discussed.

    Atomic-level cancer studies – a new challenge to distal interdisciplinary science

    Background: Nowadays, the amount of cross disciplinary research in many branches of science is growing dynamically. Multidisciplinary, interdisciplinary and transdisciplinary evaluations are being carried out. Scientific progress, in combination with the development of knowledge and new technical discoveries, allows the most complex problems to be considered in an innovative way, which results in original solutions, not present in the previous, more traditional understanding. Materials and methods: We reviewed the current data on the evaluation of pathological tissues using current mass spectrometry methods, including advanced isotope ratio mass spectrometry in the first known atomic level cancer studies. Results: The first attempts to estimate the stable isotope ratio in pathological tissues appeared as a potentially new method of understanding the biology of cancer. The changes in stable isotope concentration showed the relationship with the fundamentals of cancer – its promotion stage, and the mechanisms of its progression as well as its relationship to the standard prognostic biomarkers. The advanced multidisciplinary analysis of the results of isotope ratio assessment in cancer studies culminated in the creation of ‘The Revolve(r) Heavy Nitrogen Theory of Death’, which explains the phenomena of the proliferation force of cancer cells. Conclusions: Stable isotope profiling performed using the highly advanced and creditable isotope ratio mass spectrometry approach reveals the previously unknown area of pathological tissue biology. Overcoming interdisciplinary barriers opens a new branch of medical studies with a potential clinical application. The intriguing results of the first attempts and potential clinical implications should form the basis of the next atomic-level cancer studies projects.

    Gynaecological examination in the context of prevention – attendance, attitudes and expectations of women

    Background: A gynaecological examination, often as part of a preventive gynaecological examination (PGE), has a legislatively determined content. Despite the efforts of all those involved, women do not attend gynaecological examinations sufficiently often. The aim of this short report is to highlight the current status of women’s attendance at PGEs in Slovakia and to present women’s expectations as well as the reasons for their attendance at gynaecological examinations. Material and methods: This paper takes the form of a literature review. Results: The results of the studies suggest ways to increase women’s attendance at PGEs. The communication skills of physicians and nurses/midwives, creating a safe and intimate environment, as well as consistent education about the examination process, are considered key factors in increasing women’s participation at a PGE. Conclusion: Exploring the topic in a broader context may help to understand some of the changing aspects of women’s motivation to participate, but more importantly to understand the importance of the attitude of health professionals in gynaecological examinations.

    Mucous tumour of the appendix

    Background: In this paper we present a case of perforated mucinous neoplasm of the appendix. Diagnosis and treatment of this rare condition are briefly discussed. Material and methods: A 42-year-old patient admitted to the General Surgery Clinic for elective surgery of left-sided inguinal hernia. In the postoperative period, severe pain in the lower abdomen persisted – qualified for laparotomy, during which an appendix tumor and a large amount of gelatinous bloody contents in the right lower abdomen were found. The appendix was removed and the peritoneal cavity was cleaned in the content. Histopathological examination revealed low grade appendiceal mucinous neoplasm (LAMN) pT4a NX M1b. No angiovasia or nerve trunk infiltration were found. At the top of the appendix a perforation with mucus around was noted. Postoperative course was uncomplicated. The patient was referred to a peritoneal carcinomatosis center where a right hemicolectomy with HIPEC therapy was performed. Results: Mucous neoplasms of the appendix of low grade (LAMN) are rare tumors. Conclusions: Mucous neoplasms of the appendix of low grade (LAMN) are rare tumors. The treatment of choice is surgical excision. The most serious complication of LAMN is a rupture with the escape of mucus into the peritoneal cavity.

    Giant hepatic hemangioma with Kasabach-Merritt syndrome treated with right hepatectomy: a case report with literature review

    Background: We present a case of giant hepatic hemangioma with Kasabach-Merritt syndrome. Material and methods: A thirty-seven-year-old female presented to us with a six-month history of recurrent abdominal pain and weight loss. A CT scan was suggestive of a huge right liver mass involving the entire right lobe, which was indicative of hemangioma. Her blood chemistry showed a platelet count of 68,000 and INR of 1.4, suggestive of Kasabach-Merritt syndrome. Results: In view of the size of the mass, symptoms and associated Kasabach-Merritt syndrome, the patient was offered surgical removal of the tumour. On exploration, the tumour covered the right lobe completely and so a right hepatectomy was performed. The hemangioma was 25 cm × 20 cm × 16 cm and weighed 1.8 kg. The postoperative course of the patient was uneventful. On day 3, the platelet count had improved to 98,000 and the INR had improved to 1.2. The patient was discharged on day five. Conclusions: A right hepatectomy for a giant hemangioma of the liver is a safe procedure and can be curative for associated Kasabach-Merritt syndrome.

    Reconsidering a civil case in the same court composition when the decision making in the case may be problematic

    According to art. 386 § 5 of Civil Procedure Code where the judgment is set aside and the case is referred to be re-examined, the court examines it in the same composition, unless it is not possible or it would cause excessive delay in the proceedings. Such a provision is controversial as, in principle, the judge will have to conduct the same civil case again – even if they already gave judgement in the case. The current regulation in which the same judge is to adjudicate in the case for the second time may be seen as inappropriate in view of the content of another legal regulation. According to art. 386 § 6 of Civil Procedure Code a legal assessment expressed in the statement of reasons for a judgment issued by the court of the second instance is binding on the court to which the case was referred and the court of the second instance in the case of re-examining the case. However, this does not apply to a situation when there was a change in facts or the legal situation, or when after the court of the second instance issued a judgment the Supreme Court of the Republic of Poland expressed a different legal assessment in a resolution settling a legal issue. In the paper I pay attention to the regulation from the judges’ perspective. I present some observations on the difficult role of a judge who is to pass the judgment in the case for the second time. I focus on the issue of psychological mechanisms relating to the reconsideration of a decision that has been challenged. The paper is partly based on the interviews with Polish judges.

    Manipulacja, perswazja i socjotechnika jako formy wywierania wpływu

    Manipulację postrzega się jako strategię kierowania ludzkimi zachowaniami lub też jako metodę wpływania na ludzi przez ukryte kształtowanie świadomości bez ich wyraźnej woli i wiedzy. Niniejsze opracowanie ma na celu zaprezentowanie subtelnych różnic znaczeniowych między pojęciami manipulacji, perswazji i socjotechniki. Zadaniem podjętym przez autora jest wykazanie, że szerokie spektrum narzędzi wpływu jest różnie postrzegane i definiowane przez poszczególne dyscypliny naukowe. Inaczej interpretowane są przez językoznawców, inaczej przez socjologów, a jeszcze inaczej przez psychologów. W artykule autor przedstawia również wybrane techniki i rodzaje manipulacji oraz dowodzi, że są wśród nich takie, które szkodzą odbiorcom, ale są również takie, w przypadku których manipulujący nie ma złych intencji. Z przedstawionych analiz wynika, że wywieranie wpływu na innych ma wiele form i różni się pod względem technik, celów, intencji oraz skutków w sferze kształtowania postaw i zachowań. Co do zasady, manipulator dąży do osiągnięcia korzyści kosztem osoby manipulowanej. Takie ujęcie nawiązuje do popularnego w dziedzinie psychologii zjawiska psychomanipulacji, z jednoczesnym pominięciem jego aspektu prawnego. Tymczasem nie należy zapominać o kwestii równie ważnej, czyli odpowiedzialności karnej za manipulację, pojmowaną jako działanie szkodliwe dla ofiary i wysoce nieetyczne. Przedstawienie aspektów prawnych wydaje się zatem znaczące dla całości zagadnień związanych z przedstawianymi zjawiskami.
